Release of personal medical records
Requesting a copy of your medical records is simple, and the Release of Records team is here to help.
What you need to do:
- Download, complete and sign the Release of Records Form below.
- Email the completed form with a copy of your ID to the email address provided below.
If requesting another patients records on their behalf a copy of their ID and/or proof of legal guardianship is also required (for further clarification on this process please contact the release of records team as per contact details below)
