Phase I – In patient
Information and discussions held either ward based or within the Cardiac Rehab Centre with patients, accompanied by their spouse or a family member.
Content discussed covers pre-existing / new medical and cardiac conditions, lifestyle factors and changes, pre discharge and post discharge information, enrolment onto the cardiac rehab programme and follow up cardiac investigations, procedures, treatment and surgery, and a graduated in-patient exercise routine.
Phase II – Assessment Clinics
Appointments held at the Cardiac Rehab Centre to assess cardiac risk factors and discuss progression immediately post discharge (usually within 14 days).
literature given to support all discussions, procedures, investigations or surgery, healthy heart wellbeing and post discharge information.
24 Hr. Cardiac Rehabilitation Helpline:
Cardiac Rehabilitation information and support service which can take live or recorded calls from patients, relatives and staff.
Calls and emails are replied to only during 9am-5pm (excluding weekends & bank holidays).

Phase III – Cardiac Rehabilitation Programme.
Outpatients cardiac rehabilitation comprehensive programme, composed of structured learning via group orientated discussions, prescribed exercise circuit training and stress management /relaxation sessions. Service Users wishing to enrol need to contact either their the Cardiac Rehab Manager or speak to their hospital doctor or GP for a referral.
Enrolment clinic preceding entry to the Cardiac Rehabilitation Programme.
Clinics held at the end on completion of the Cardiac Rehabilitation Programme.
1st & 2nd Annual Review Clinics
Assessments conducted at year 1 and year 2 after successful completion of the cardiac rehab programme.
Allows the staff to assess for any improvements or deteriorations in lifestyle up to two years post- programme completion.
Cardiac Rehab Self Help Groups
Exercise groups run by group leaders of the local cardiac support group “Gibraltar Cardiac Association” for patients who have successfully completed the Cardiac Rehabilitation Programme to enable these patients to continue to exercising via group format every week.
Innovative new service commenced in September 2006 which received a prestigious award from the BJN in 2015 and which continues to operate within the Cardiac Rehab Centre premises.

CR Self Help Group
Self run group of service users who have successfully completed the Cardiac Rehabilitation Programme and continue to exercise as a group long term. Innovative new service commenced in September 2006 and which runs very successfully since.
