Phase I – In patient
Information and discussions held either ward based or other within the hospital facility with patients, their spouse or relatives. Information discussed covers pre and post expectations and follow up in relation to cardiac investigations, procedures, treatment and surgery.
Phase II – Immediate pre discharge
Local literature given to support all discussions, procedures, investigations or surgery, healthy heart wellbeing and post discharge information.
24 Hr. Cardiac Rehabilitation Helpline:
Answerphone service which takes calls from patients, relatives and staff.
Calls are replied to weekdays only 9am-5pm excluding weekends and bank holidays. Tel: +350 20007203
Phase III – Cardiac Rehabilitation Programme.
Outpatients cardiac rehabilitation comprehensive programme, composed of structured learning via group orientated discussions, prescribed exercise circuit training and stress management /relaxation sessions. Service Users wishing to enrol need to contact either their the Cardiac Rehab Manager or speak to their hospital doctor or GP for a referral.
Enrolment clinic preceding entry to the Cardiac Rehabilitation Programme.
Clinics held at the end on completion of the Cardiac Rehabilitation Programme.
CR Self Help Group
Self run group of service users who have successfully completed the Cardiac Rehabilitation Programme and continue to exercise as a group long term. Innovative new service commenced in September 2006 and which runs very successfully since.
